Judge Victor Caruana Colombo has been appointed by the Prime Minister to lead an independent inquiry into the death of AFM Gunner Matthew Psaila in a training accident last week.

The board of inquiry, which also includes Brig (retired) John Spiteri and Joe Sultana, former Director-General of the Department of Civil Aviation, will investigate the operations of C (Special Duties) Company, of which Gunner Psaila formed part.

Gunner Psaila would have been 20 today. He died three days after an accident at Chadwick Lakes where he was submerged in water for some 10 minutes.

Earlier, it was announced that the AFM had appointed Lt Col Mario Borg to head an internal inquiry into the death of Gunner Psaila. It will also look into health and safety procedures as applicable to the military.

The Commander, AFM ordered the inquiry to report on the sequence of events leading to the tragic death and whether safety procedures were observed at all times.

Lt Col Mario Borg is the commanding officer of the Third Regiment which is not being investigated. Each regiment in the AFM has its own safety officer.
